Aetna - Simple Steps to a Healthier Life® BlueCross Blue Shield of Georgia - 360 Health HealthStatus - Health Risk Assessment You will be asked to provide information about your family history, your health and your habits. To receive the most accurate results, you will need information such as your blood pressure, cholesterol and weight. After submitting your personal information, you will receive a detailed health report that can be shared with your health care provider. HealthQ is Step UP Emory’s promotion of health questionnaires, which are designed to help you identify your risks for developing various health conditions, and may discuss medical and family history, eating habits, physical activity, stress, safety, health interests and more. The questionnaire is followed by helpful tips to help you reduce or eliminate those risks. This questionnaire is designed to give you tools to help you take charge of your health and make positive lifestyle changes. Return to FAQs Everyone! Faculty and staff with benefits through Emory are eligible to participate through their health insurance vendors. If you are enrolled in another plan, please take the time to complete your vendor’s health questionnaire. While voluntary, the questionnaire may raise your awareness of risk factors, and set you on the path to overall wellness. How long will the questionnaire take? What will I need to complete my HealthQ? While not necessary, some questionnaires may request the following information to provide you with a more accurate report: Medical History (Personal and family) Weight and Height (shoes off) Body Mass Index Blood Pressure Cholesterol levels Blood glucose If you do not have this information readily available, you may want to contact your doctor or visit a Comprehensive Screening Clinic provided by the Faculty Staff Assistance Program. Return to FAQs What happens after I complete my HealthQ? After completing the survey, you will receive a confidential, personalized report to help you better understand your health risks and needs. The questionnaire will score and rate overall behaviors as well as health in key areas. Suggestions for improvement are generally linked to specific outcomes. Will my information be kept private? Yes. Your personal information will reside with the health questionnaire provider, through a password protected system. Emory will not have access to your personal information. Return to FAQs
What should I do with the information I receive? Your questionnaire will be filed with your personal medical information as well as recommended courses of action for improving your health.
