Family Medical Leave (FMLA)

Eligible employees may take up to 12 workweeks of unpaid, job-protected leave under the Family and Medical Leave Act (FMLA) in a rolling 12-month period for specified family and medical reasons.

Submit requests and view status for FMLA or Medical Leave online.  Employee’s supervisor/manager and departmental Human Resource Representative are immediately notified via e-mail when a request is submitted.  Employees receive e-mail notification when leave has been approved. The online request form is also available through Employee Self Service.

Employees requesting personal, educational or military leave should use the Non Medical Leave Request Form.
